Policies of the practice

 
Door To Door Doctors is a traditional solo Family Medicine Practice.  My practice style is shaped by my obligations to practice medicine well and to take good care of myself and my own family.

After hours coverage:

The main office line will give you an option of leaving a message on any line.  There is an after hours emergency line that comes straight to Dr London that is for medical emergencies only.  If you are ill, do not hesitate to call early in the course of the illness.  This helps to plan in the event that you need an appointment.  If you or your child are severely ill after hours, particularly if you have chest pain, trouble breathing or have symptoms suggestive of a stroke, do not hesitate to call.

 
No telephone refills if at all possible.  By the end of every office visit, we should have made a clear plan with regard to follow-up.  The nature of an illness, whether acute or chronic, determines the length of time to the next appointment.  For example, diabetics generally need to be seen at 3-month intervals.  People with high blood pressure that has been well controlled for years on the same medications might need to come in only every 6 months, while a patient with newly diagnosed hypertension not yet well controlled might need to come in every week.  I try to ensure that I write prescriptions for enough medication to last until the next visit.  The need for phone in medication refills, therefore, should arise rarely, if ever.

 
If your prescription requires a Prior Authorization I will only work with pharmacies that do their job and start that process. These include Hannaford, Milbridge Pharmacy and Walgreen’s some Rite Aids and now Machias Friendly (maybe).  I will NOT do a pharmacists’ job for them as I have enough to do.  
 
 I would prefer to do minimal telephone medicine – exceptions are asthma flares in patients with clear asthma management plans, and urinary tract infections in otherwise healthy women.  I am happy to answer questions via the patient portal, please do not abuse this, as it takes away from my time with patients in the office and my family. The exception to this is Direct Primary Care patients who do have access to virtual visits.

 

Missed Appointments:

 
Missed appointments: appointment time is precious, and the time of your appointment has been reserved for you.  “No shows” diminish our ability to make time available to acutely ill patients.  For this reason, we have adopted the following policy:



•First missed appointment: you will receive a reminder call and may reschedule if necessary.
•Second missed appointment: you will be charged for the full cost of the visit (this is not covered by insurance)
•Third missed appointment: you will be charged for the full cost of the visit, and potentially discharged from the practice.

 
•First time you Reschedule/cancel with less than 24 hours notice: we will give you a warning
•Second time you Reschedule/cancel with less than 24 hours notice: we will charge for the full cost of the visit if we cannot fill the time slot.


•First time you arrive >15 min late for an appointment you will be given a warning &  may  reschedule for another time. Dr London will try to accommodate seeing you that day after patients who are on time.
•Second time you arrive >15 min late for an appointment you will be charged $35 for the appointment and may reschedule for another time. Dr London will try to accommodate seeing you that day after patients who are on time.
•If you are habitually >15 minutes late you will not be given your own time for an appointment but a floating time and potentially discharged from the practice.

 
Your insurance is a contract between you (sometimes your employer) and the insurance company.  We are not a party to that contract. If your insurance changes, you are responsible for giving us that information.  We should not have to pull it out of you.  As a courtesy, we will file an insurance claim for you ONE TIME ONLY per office visit.  After that, the payment is your responsibility to us, and we will provide you with a form for you to be reimbursed by your insurance company. 

 

Checks returned for insufficient funds will incur an additional charge of $60.00.
Payments not received within 30 days of billing will incur interest.
